NORTHWEST COLORADO COMMUNITY HEALTH PARTNERSHIP IS STRIVING TO CREATE AN INTEGRATED HEALTHCARE SYSTEM IN RIO BLANCO, MOFFAT, ROUTT, JACKSON, AND GRAND COUNTIES
